The Health Care facility at The Forum is wonderful. My Mother, Tanny Gustafson, was there for seven months. Staff REALLY cares and I had the feeling that each person cared about Mother as though she were their mother. The facility is up to date and clean. The food is excellent; the atmosphere is upbeat; everyone is very accommodating and helpful. Entertainment head Connie is wildly creative and is constantly upbeat. She comes up with fun activities and creates a friendly family like atmosphere where group participation keeps the days lively. The administrative staff is efficient and prompt to make any improvements they can. I highly recommend The Forum.

My father passed away at The Forum Healthcare Center at Knightsbridge, mid September of this year, 2018. He was there for about 4 1/2 months. He was almost 96 years old (3 weeks shy of 96th bd). Before this, he lived in his own home. I believe we should always be on top of things and be advocates for our people wherever they are being cared for. Listen to them and, out of respect, when possible, don't think they are just too old or ill to still have an opinion or option for their own care. After all, we may all be in this situation one way or another someday. I say this because no one really wants to leave their home and go to a nursing home, and no health care situation is perfect. There will be incidents occasionally. That being said, The Health Care Center at the Forum was very nearly perfect! From the dining and kitchen staff, housekeeping, reception, aides and nurses, to the administrators, everyone was great. They all pitched in, just like a big family. Also, every aspect of this facility was easily accessible for residents, even getting out on the porch to sit in the sunshine. But....the activities director, Connie Fellers, was the one who really kept the residents going; scheduled all of the wonderful activities, got them out of their rooms and kept them occupied, their minds and bodies busy, and smiles on faces most days. My Dad always called Connie his biggest fan and cheerleader. He was always amazed by her energetic enthusiasm. My family also appreciated and grew to know and love other residents and their families during the months Dad was there. After being there every day, It was a real adjustment to suddenly not see them anymore. The bottom line is that the Health Care Center at the Forum gets 5 stars ***** from my family. Connie Kelch

Lovely facility, nicely furnished & immaculately clean. Comfortable & welcoming for patient& family. But what makes the operation outstanding is a remarkable staff. Administrative, social services, activities, medical, nursing, therapy, tech, housekeeping, food service personnel ALL unfailingly competent, helpful & kind. Thanks to all!

I wanted to share quickly my thoughts on a 2 day visit from St Augustine, FL this past weekend. I am extremely impressed and satisfied with activities director Connie and her upbeat attitude and interaction with the residents of long term care side. She loves people and I can tell she cares deeply of the residents and the programs at The Forum. The picnic was fun and guitarist Matt Frampton rocked the house. Connie adds a bright spot to an unpleasant situation concerning the downturn of our loved ones. I left feeling my Mother is in good hands thanks to Connie and the cook I met at our picnic.... great people!

I truly feel blessed to be here as a full time resident. Since all of my furniture fits in my rooms, I truly feel like I’m home. I also feel so spoiled not having to cook meals or clean anymore. Each of the staff members are so very kind as are the maintenance crew.

Staff were friendly, caring, knowledgeable and efficient. The facility was clean and tastefully decorated to feel homey. Rooms were large with appropriate lighting and seating for guests. Food options were a nice addition to services provided and dining room staff were accommodating to special requests. Administration was “top notch,” professional, and caring.
